Patient Portal Access – Once you have received your PIN, access the Patient Portal following the steps below.

Registration Instructions: 1) Browse to http://patientportal.intelichart.com 2) Select the tab to “Create an Account” 3) Fill out the personal information including the Patient Portal access PIN, read & accept the Terms and conditions then select Complete.
