Please note: UPPC is an Active Pediatric Clinic Practice and unable to accommodate WALK-INs or phone calls on Covid Vaccine Inquiries During Clinic Hours. Clinic hours are from 9:00 am to 5:00 p.m.
To receive the vaccine you must 1) be eligible, 2) be pre-registered, and 3) have an appointment.
You can print a pre-registration form from this website or pick one up from just inside our waiting area.
You can drop off completed application/registration forms at the front desk or email them to firstname.lastname@example.org. Another option, if you are unable to print, is to provide the required details in an email while requesting an appointment at the same time. Please include NAME, Date of Birth, ADDRESS and a PHONE NUMBER where we can reach you to schedule your appointment.
Bring your insurance card(s) with you to your appointment and we will take a copy at that time.

All COVID Vaccine’s must have an appointment. We do not provide WALK-IN COVID Vaccinations.
COVID Vaccine appointments are made in the order we received your registration form and appointment request which we have more than 1200 at this time and are working through them as quickly as possible.
People with no insurance are still welcome! You will not be charged for a COVID-19 vaccine or administration fee but we are required to verify your information.
Please note, you will receive a call from 253-564-1168 or 253-564-1115 when we are ready to schedule. If you miss our call you could miss your opportunity to fill an open appointment.
