UPDATE – COVID-19 Vaccines

Update as of 2/26/2021 – UPPC was NOT allocated vaccine again this week. We will know on Friday, 3/5/2021, whether UPPC has been allocated vaccine and update this page once we receive confirmation either way.

Our clinic has been working diligently to be able to provide Covid Vaccine to our community. After being approved and listed as a vaccine site by the DOH, we have not yet been provided any vaccine.

Registrations already sent in will be kept on file. We will contact you as soon as we receive an update from the DOH. Unfortunately, at this time, we must pause on accepting new registration forms until we have heard from the state as it appears they are moving in a different direction than was previously explained during the application process.


If the DOH does provide our clinic with COVID vaccine, we still plan to vaccinate anyone who falls within the state guidelines for the current phase we are in.  People do not have to be patients of University Place Pediatric Clinic to be vaccinated. 

Please DO NOT CALL OUR OFFICE. This message is provided as instructions for you to help us make this process smoother for you and for our clinic staff. Please read the message below carefully.

If you, or someone you know would like to receive the COVID-19 vaccine we will need to get you/them registered in our system. Please print out the forms below, fill them out, and return them to our office.  You can email completed forms to nurse@uppediatrics.com or drop them by our office.  You also can pick up a packet with the forms in our office lobby.  We will need a copy of an insurance card (a picture taken with your phone is OK to include).

No insurance and under-insured are still welcome! You will not be charged for a COVID-19 vaccine or administration cost but all other documents are required to be completed.

Once we receive confirmation as to the number of vaccine doses we are going to receive and the date, we will call and schedule you for an appointment. 

Please note, you will receive a call from 253-564-1168 when we are ready to schedule and be prepared to answer the call.

Please know, as of 1/21/2021, the current phase guidelines from the state for vaccinating for COVID is 1B.  That means anyone over the age of 65 can receive the vaccine.  Also in this category are people over the age of 50 who live in multi-generational households.  We are also able to vaccinate anyone who falls into the 1A category as well, this includes anyone who works in healthcare, first responders and any long-term facility residents.

We are committed to assisting in vaccinating our community.  Our goal is to vaccinate as many people as possible who want to be vaccinated.  Please be patient with us as we move through the phases that have been set forth by the state.  If you would like to be vaccinated but you do not currently meet criteria, we encourage you to complete the paperwork and turn it in to our office to expedite the process once you are able to be vaccinated. 

**Please note, at this time COVID-19 vaccine is not approved for anyone under the age of 16/18 depending on the manufacturer of the vaccine**
